When signing off the questionnaire, via the email from [email protected], you will see a screen like this;

Potential OTP (one-time passcode) issues during the declaration process may arise and cause inconvenience. Common scenarios include encountering an OTP prompt or errors completing the OTP system, which can hinder the signing process.

The OTP is not needed to submit the form. If you just press "Confirm" the declaration will then be signed.

If an OTP prompt appears in the email, it is not necessary to enter the OTP before proceeding. Clicking 'Confirm' will bypass the OTP step while successfully completing the submission.

If OTP errors persist and prevent the completion of your declaration, export the filled questionnaire and use it as an attachment when contacting IASME. Provide an explanation of the issue to enable manual processing or further support.

If you are wanting to review the questionnaire and answers, please contact [email protected]

For technical assistance with OTP-related issues, consider directly reaching out to IASME.
